Dear Brother or Sister in Christ:

Thank you for your decision to serve the Lord here at Calvary Chapel.

The desire to serve is a natural and proper response to the work of Christ within you. Where to serve becomes the big question. This Ministry Questionnaire has been designed to help us to get to know you better and to assist you in determining your place in the Body of Christ. Each of us has a unique blend of, talents, traits, gifts, and past experiences that God will use to accomplish His work.

The Apostle Paul uses the “body” as a metaphor to describe how the local church should function. Each one of us is a unique part of the body of Christ, and just like our human bodies each part has a specific purpose. One thing we are attempting to avoid is having you serve in an area of ministry for which you are not designed. One of the main reasons for “burnout” in ministry is the lack of a proper “fit.” Taking your time to fill out the Ministry Questionnaire will reduce the chances of placing you in the wrong area of ministry and maximize the potential for you to experience one of the greatest joys you’ll ever know - being used by God in the capacity for which you were designed.

Ephesians 2 tells us that we are God’s workmanship created in Christ Jesus for good works, which He planned for us before the foundations of the earth. Enjoy the excitement and challenge of this truth as you complete the enclosed Ministry Questionnaire and find the destiny of your mission for God. Then, have a blast serving with all your heart. Serving others is a wonderful way to use your gifts and talents for the glory of God. We are thankful for those who come along side of us in meeting the needs of the body that God has called us to. May your service be joyful and rewarding as you answer the call God has upon your life.

You might be surprised to see a background consent and waiver form which we have asked you to complete. Please let me encourage you not to let this form intimidate you. Because of the area of ministry you desire to serve in this is a necessary requirement and serves for the protection of everyone involved.  Trust God and He will prove faithful in placing you exactly into the area for which He designed you in the Body of Christ.
